First Concern
Step 1: Discuss problem with parent
Fill in 1st Concern Statement
Ask parent for support
Collect evidence in 1st Concern Statement
Agree homework period of 4-6 weeks
Provide a differentiated curriculum in class
Step 2: Monitor differentiated curriculum & homework for 4-6 weeks, then decide action

Continued Concern: School Action (LSA Support) - Fill in School Action Discussion Statement for course of action.
- Child to have differentiated curriculum in classroom and an I.E.P. with support from LSA
- * I.E.P. is the responsibility of the teacher to be written in S.E.N. file and should show realistic targets set by the teacher
- Teacher to provide weekly planning for LSA to include learning objective and success criteria
- LSA to write individual comments on each child each session on planning sheets to inform teacher for future planning.
Reporting to Parents
- Half termly progress report meeting to be held with parents. Fill in Discussion Record each time 5 photocopies (see list below).
- Fill in Review I.E.P. at end of every term, before end of term parents meeting. At review meeting show parent programme of work child has been following and discuss review. Use LSA lesson plan as evidence for review.
- Complete review decision.
- Photocopy DR for:
- Parent
- Class record file
- SEN folder (LSA will update Sims from this copy)
Repeat from * as long as child is on School Action.
Further Concern - School Action Plus
- If child shows no significant improvement with LSA intervention, then call in an appropriate outside agency. (External Specialists)
- If problem continues, consider whether the child should be referred for statutory assessment (Stages 4 & 5)
